Basically, we're looking for there to be some sort of angst or upset over what happens. It doesn't matter if it's an illness or an injury - or even something emotional like grief or guilt or a serious breakup or bout of mental depression. The idea is for the reader to get off on the trauma of the sufferer and his/her loved ones. Because we are sick that way o_O
So I mean, Neal could be going in for a tonsilectomy, but if there are serious complications, or maybe he could lose his voice, and you play up the emotional upset that scenario would bring, then I'd say you've successfully whumped your man!
