This is a topic that’s a little more serious…it is a little more grounded in reality than standard Hurt/Comfort, in that there are not easy fixes and miracle cures. It’s not what’s on TV, for the most part. And that’s what gives it its appeal, I think, and how fandom can do what it does best – reflect a reality with the characters that we don’t ever really see – in canon or on our TV screens.
 
Because in real life, when someone gets shot through the shoulder, there’s likely to be serious nerve and muscle damage they will have to deal with for the rest of their lives. And in real life, people aren’t magically cancer-free at the end of the story arc – those who have survived it must live with the after effects for a very long time.

Actually, just google "heart murmur" and you'll find a ton of stuff.

Complications of a heart murmur can be chest pain, unexplained breathlessness, fatigue or fainting, heart palpatations.

While there are "innocent" heart murmurs which usually occur in children and which they grow out of in time, Neal is not innocent,as we all know.

Heart murmurs in and of themselves can be a symptom of developing heart disease, a hole in the heart, a leak in a heart valve, a narrow heart valve, susceptibility to an infection of the heart (endocarditis).

The thing is the heart murmurs in adults are generally very early signs of other more serious conditions, but if treated at the onset, can prevent those other conditions or at least mitigate them.

Neal may have had a heart murmur as a child and thought he grew out of it, as most children do. But lets face it, he's going through stress like few people ever have and so that could bring it back, making him more vulnerable to more serious complications.

Well, of course I did google heart murmur and look into various conditions from there, but I didn't find anything telling me it was plausible for a congenital heart murmur that didn't need treatment to cause the kind of cardiac symptoms you're talking about. I did see the risk of endocarditis, but the symptoms of that didn't seem to be what you're looking for. I like to be able to find articles telling me the likely causes/symptoms/complications/progression of a condition before I write about it, so I thought you might know of a more specific link.

Anyway, I might do more research later on to see if I can find something solid to hang this on because I really do like the idea.
